Why Decision-Making Changes Under Extreme Stress

You can know exactly what to do in a calm room.

Then the alarm sounds.

Time shrinks. Noise becomes louder. Your heart rate rises. One piece of information captures all your attention while another disappears.

The choice that seemed obvious yesterday suddenly becomes difficult.

This is why the sentence “I would just…” is one of the least reliable things people say about emergencies.

Stress does not simply add emotion to an otherwise unchanged brain. It changes the conditions under which decisions are made.

But the popular version is also too simple.

Stress does not make everyone stupid. It does not erase training. And high arousal does not produce one universal pattern of failure.

The real science is about which cognitive processes change, when, and how experience can alter the result.

The body is trying to solve a different problem

Acute stress activates systems designed to mobilize action.

Attention shifts toward what appears immediately important. The body changes energy use. Heart rate and arousal increase. Stress hormones and catecholamines alter brain function.

From an evolutionary perspective, this is not irrational.

If a threat is immediate, endlessly comparing twenty options may be worse than rapidly selecting a workable one.

The problem is that modern emergencies can demand both speed and complex reasoning.

A fire officer, emergency physician, pilot, parent or dispatcher may need to act quickly while also tracking multiple variables.

That is where stress can become cognitively expensive.

Working memory becomes more fragile

Working memory is the limited mental workspace used to hold and manipulate information.

Remember the address. Track who has been accounted for. Compare two routes. Keep the instruction in mind while performing the next step.

A meta-analysis of acute stress and executive function found that acute stress impaired working memory and cognitive flexibility on average.

The word average matters.

Effects vary across tasks and people.

But the general pattern helps explain a common emergency experience: the person can still think, but the number of things they can think about effectively at once becomes smaller.

This is why checklists and simple procedures can be valuable. They move part of the task outside the head.

Cognitive flexibility can narrow

Cognitive flexibility is the ability to change strategy when circumstances change.

Under stress, people can become more likely to persist with the first interpretation or action pattern.

That may look like continuing down a blocked route, repeating an action that is no longer working, focusing on one threat while another becomes more important, or struggling to abandon the original plan.

Again, this is not inevitable.

But it is one reason emergency planning should contain decision points, not just instructions.

“If X happens, reassess.”

A plan that cannot change is not resilience. It is rigidity.

Tunnel vision is both metaphor and mechanism

People under threat often describe tunnel vision.

Sometimes they mean literal narrowing of visual attention. Often they mean something broader: one problem dominates consciousness.

A parent focuses on finding a missing phone while ignoring the building alarm. A driver stares at the obstruction and misses the open path. A team becomes fixated on the first diagnosis.

Stress increases the salience of certain cues.

That can be useful if the cue is the true threat. It can be dangerous if the cue is only the most emotionally powerful thing in the room.

Stress does not affect every decision in the same way

A 2024 systematic review examining biological stress systems and decision-making found evidence that acute stress can impair multiple decision processes.

But research across the field is heterogeneous.

A separate 2024 meta-analysis, for example, found no overall effect of acute stress on monetary delay discounting.

That is a useful correction.

“Stress changes decision-making” is defensible.

“Stress always makes people more impulsive” is too strong.

Different tasks recruit different systems. Stress interacts with context, experience, timing, controllability, fatigue and the specific decision being made.

Expertise changes the picture

Research on high-risk professionals is especially interesting because these people routinely make decisions under pressure.

A systematic review of decision-making in healthcare, military, aviation, fire and rescue environments found repeated use of recognition-primed decision-making.

Experienced professionals often do not generate a giant list of options.

They recognize a pattern. They retrieve a plausible action from experience. They mentally test whether it fits. Then they act.

This can be extremely efficient under time pressure.

It also explains why experience is more than factual knowledge. Experts possess libraries of patterns.

Intuition is not magic

Recognition-primed decisions are sometimes called intuitive.

In this context, intuition does not mean a supernatural signal.

It means rapid pattern recognition built from prior exposure and feedback.

A veteran paramedic may notice that a scene “doesn’t feel right” before consciously listing every cue.

The feeling can be meaningful because the brain has learned patterns.

But intuition is only as good as the environment that trained it.

If a domain provides poor feedback, rare events or biased experience, confidence can outrun accuracy.

Under stress, intuition can save time. It can also lock the person onto a mistaken pattern.

Why drills work

Training does not make stress disappear. It changes what happens when stress arrives.

The NIOSH literature on emergency decision-making has long emphasized the relationship between prior training and performance under stressful conditions.

A drill can reduce novelty. It can turn an abstract instruction into a practiced sequence. It can expose missing information.

Most importantly, practice can lower the number of decisions that must be invented in real time.

The dangerous myth of “rising to the occasion”

People like to imagine crisis reveals hidden competence.

Sometimes it does.

But performance under pressure is usually constrained by what existed before the pressure: knowledge, practice, physical condition, team communication, sleep, experience, clear roles and available information.

A crisis can produce extraordinary behavior.

It should not be your training plan.

Simple does not mean stupid

Under cognitive load, simplicity becomes a feature.

Short instructions. Clear priorities. Visible checklists. Defined roles. Pre-agreed communication channels. One next action.

This is not because people under stress become incapable.

It is because every unnecessary mental operation competes for limited attention.

The goal is not to prove how much complexity a person can hold under pressure. It is to design the system so they do not have to.

The pause problem

“Stop and think” can be excellent advice. It can also be dangerous if immediate action is required.

The better principle is: match the thinking style to the time available.

When seconds matter, use practiced actions and clear rules.

When minutes are available, broaden the scan.

When the situation stabilizes, deliberately challenge the first interpretation.

High-risk decision research suggests professionals move between recognition-based and analytical strategies depending on time pressure and uncertainty.

Good decision-making is not always slow. It is adaptive.

Teams can protect the individual brain

One person under stress has one attention system.

A team can distribute cognition.

One person monitors. Another communicates. Another watches the environment. Another tracks resources.

This works only when roles and communication are clear. Otherwise the team creates additional noise.

The same principle applies in families. A simple division of tasks during an outage or evacuation can reduce duplicated effort and missed needs.

After the immediate danger, cognition still matters

Stress does not end when the event becomes less dramatic.

Sleep deprivation, uncertainty, heat, hunger, repeated alarms and emotional exhaustion can accumulate.

A person may make reasonable decisions in hour one and poor decisions on day three.

Resilience therefore includes recovery: rest, rotation, food and hydration, accurate information, reducing unnecessary alerts and reassessment.

The human decision-maker is part of the emergency system.

That is unrealistic.

The lesson is: build systems that still work when calm is incomplete.
